Health Guarantee and Sales Contract
As referenced herein, the parties to this agreement are identified as follows:

This sales agreement is acknowledged and confirmed as being a complete description of the terms and conditions of the sale concerning the following puppy and purchase price:

This puppy is guaranteed to be in good health and has been examined by our licensed veterinarian at seven weeks of age. Seller warrants at time of pickup the puppy referenced above is in good health and is free of any congenital defect that causes death, natural or via euthanasia or significantly impairs the quality of life. BUYER must have puppy examined by a licensed veterinarian within 72 hours of pickup, or this health guarantee is NULL and VOID, with no warranty of health, whether expressed or implied by law and is hereby voluntarily waived by BUYER. All veterinarian fees or associated costs are the responsibility of the BUYER. There is no guarantee against fleas, worms, ear mites, coccidia, giardia, due to SELLER having no control over puppy’s new environment. Every precaution has been taken to ensure you a healthy puppy at pickup.Parvo: There is no guarantee against Parvo. Your puppy has been vaccinated at seven weeks, and must continue to receive booster vaccinations per the recommended veterinarian schedule to receive full immunity. Please use common sense when taking your new puppy out.
AKC registration: Seller is providing application for registration with American Kennel Club. Registration of puppy is at the expense of the BUYER. SELLER is providing you with a pet quality puppy, with Limited Registration. BUYER agrees this puppy will only be registered with American Kennel Club, also known as AKC.
Breeding conditions and Restrictions: This puppy is being sold with the understanding that he/she is to purely be a pet. Only after an agreement between the BUYER and SELLER, and under certain conditions, will the breeding of the adult dog be allowed. This includes the dog passing appropriate health clearances (hips, elbows, heart) and documentation is provided by the owner to the breeder, then and only then, will the registration be opened to allow breeding of this dog. A fee of $5000 will be collected before registration is opened. If it has been discovered that the BUYER has bred the dog and produced a litter without an agreement with the SELLER, a fine of $5000 per puppy will be enforced and collected.
Spay/Neuter: BUYER agrees to not alter this puppy prior to 12 months of age, or any health guarantee will be voided.
Food: It is Required that this puppy continues for a minimum of 60 days on TLC Pet Food to avoid stomach upset.
INITIAL _____NuVet/NuJoint: The standard health replacement guarantee is six months from pick up date. This guarantee can be extended to twelve months from date of pickup by demonstrating proof the pup was supplemented with NuVet and NuJoint, purchased thru code 98860. Continued supplementing is highly recommended to maintain healthy growth of joints and immunity. www.nuvet.com or call (800)474-7044
First right of refusal: If at ANY time, BUYER cannot or does not want to keep this puppy, SELLER has the right to first refusal; there will be no questions asked. If first refusal is denied to SELLER, then SELLER has the right to demand the entire price of the puppy refunded back by BUYER to SELLER plus a penalty of $1,500. The puppy must be in good health, and BUYER must provide proof of current shots, wormings, and rabies vaccines. All AKC paperwork must also be returned to SELLER. This puppy will never be turned over to an animal shelter or animal rescue. If the puppy is discovered to have been turned over to a shelter or rescue, a fine of $10,000 will be collected from the BUYER.
Housing and Care: This puppy will be kept at proper weight as per AKC standard, and never allowed to become obese. This dog will be given adequate and proper housing and shade, as well as exercise daily. This dog shall be given the proper amount of food and have fresh water available at all times. In no manner shall this dog be mistreated or neglected in any way. The dog shall receive adequate and proper vet care and all vaccinations and heartworm preventative must be maintained and up to date. This puppy will be subjected to moderate exercise not extending 15 min a day for the first year.
Other– There is no guarantee offered to adult size, color, hair texture, temperament, or dropping of testicles. THIS GUARANTEE IS NON-TRANSFERRABLE AND APPLIES TO ORIGINAL PURCHASER ONLY. THERE IS NO EXCEPTION TO THESE POLICIES.SELLER WILL NOT ACCEPT PUPPY CHECKUPS FROM BANFIELD CLINICS ASSOCIATED WITH PETSMART STORES.
THIS SALE IS FINAL, AND DEPOSITS ARE NON-REFUNDABLE.
